Leptin Resistance Makes Hunger Harder to Trust

Some people walk around with plenty of stored body fat and still feel hungry all the time.

That sounds like a contradiction until you understand leptin.

Leptin is one of the main hormones involved in appetite and body-weight regulation. It is produced by fat cells, and in a normal system, more stored fat generally means more leptin in the blood. That leptin travels to the brain and helps communicate that the body has stored energy available.

In plain English: leptin is one of the ways your body tells your brain, “We have energy stored. You do not need to keep eating right now.”

Very helpful. Very elegant. Biology occasionally has its moments.

The problem is that many people carrying more body fat do not have too little leptin. They often have plenty of it. Sometimes a lot of it. The issue is that the brain is not responding to the leptin signal properly.

That is leptin resistance, and it changes how we should talk about hunger.

Hunger is not just a character test

The usual public conversation around hunger is embarrassingly shallow.

If someone overeats, the assumption is that they lack discipline. If they snack constantly, they must have poor self-control. If they regain weight after dieting, they “fell off track,” which is a polite way of saying we think they failed but would like to sound encouraging while judging them.

This is where appetite biology matters.

Leptin is released by adipose tissue, and leptin levels generally reflect how much stored energy the body has. In a well-functioning system, rising fat mass leads to higher leptin, and higher leptin should reduce appetite and support normal energy expenditure. The brain receives the message that energy stores are sufficient.

But in leptin resistance, that message does not land properly. The person may have significant stored energy, but the brain behaves as if the signal is weaker than it should be. Appetite stays higher. Cravings feel harder to ignore. Food becomes more mentally present than it should be.

This does not mean the person is helpless. It means the internal pressure to eat is not imaginary.

That distinction matters because the wrong explanation creates the wrong plan. If hunger is treated only as a discipline problem, the answer becomes more restriction, more guilt, and more “just push through.” If hunger is treated as a biological signal, the better question is: why is the signal so loud?

Why more body fat does not always mean less hunger

On paper, leptin should make weight regulation simple.

Fat cells produce leptin. More fat cells, or larger fat stores, generally produce more leptin. Higher leptin should tell the brain there is enough energy available. Appetite should come down.

But many people with obesity have high leptin and still experience strong hunger. That is the uncomfortable part. The body is producing the signal, but the brain is not responding to it properly.

This is similar in pattern to insulin resistance, although the mechanisms are different. In insulin resistance, the body can produce more insulin because the cells are not responding well to it. In leptin resistance, the body can have high leptin because fat mass is high, but the appetite-regulating parts of the brain do not respond as clearly as they should.

So the problem is not always “not enough hormone.” Sometimes the problem is poor response to the hormone already there.

That is why simplistic weight-loss advice misses so much. Telling someone with distorted hunger signals to “just eat less” is technically possible in the same way telling someone with a broken sleep schedule to “just wake up refreshed” is technically a sentence. It does not explain the mechanism, and it does not make the behaviour easier to perform.

Leptin helps explain why hunger can feel out of proportion to actual energy need.

Leptin does not explain everything

This is the part where the internet usually ruins a useful idea.

A hormone gets attention, someone turns it into The One Thing, and suddenly every problem has a neat little villain. Cortisol did it. Insulin did it. Now leptin gets dragged into the same circus, wearing a tiny top hat and being blamed for everything from cravings to your inability to ignore supermarket pastries.

Leptin is important, but it is not the only thing happening.

Insulin response still matters. Blood sugar still matters. Sleep still matters. Stress still matters. Food quality, meal timing, movement, protein intake, and ultra-processed food exposure all affect appetite regulation. The wider hormonal environment matters too.

Leptin sits inside that system. It is one major regulator of appetite and energy balance, not a magic button you press directly.

That is important for coaching because the practical target is not “hack leptin.” The practical target is creating conditions where appetite becomes easier to regulate. Stable meals. Enough protein. Fewer ultra-processed foods. Better sleep. Better glucose and insulin control. Less constant grazing. A plan that reduces weight without turning every day into a small psychological hostage situation.

You do not need to micromanage leptin directly to respect what leptin is telling you about the system.

Weight loss can make hunger louder

There is another side to leptin that people often only notice once they start losing weight.

When fat mass falls, leptin usually falls too. That makes sense because fat cells produce leptin. Less stored fat generally means less leptin.

The brain can interpret that drop as an energy-deficit signal. Appetite can increase. Energy expenditure can come down. The body becomes more interested in food at exactly the moment the person is trying to eat less.

This is one reason aggressive dieting often gets harder over time.

In the first week or two, motivation can carry the plan. The scale moves, the person feels in control, and everything looks beautifully simple. Then hunger rises. Food thoughts increase. The same meals stop feeling satisfying. The person starts negotiating with themselves in front of the fridge like a tired lawyer.

That is not always weakness. Often, it is biology responding to weight loss.

This is why the structure of the plan matters. If the approach relies only on constant restriction, the person eventually has to fight increasing hunger with decreasing mental energy. That is a bad trade. A better plan improves satiety, reduces glucose swings, controls meal timing, protects protein intake, and supports sleep so the person is not trying to run the entire process on willpower.

Willpower has a role. It is just a terrible foundation for a long project.

What this changes in practice

The point of understanding leptin is not to obsess over a hormone you cannot feel directly.

The point is to stop treating hunger as automatically trustworthy or automatically shameful.

Sometimes hunger is useful. It can tell you that you under-ate protein, slept badly, ate too many fast-digesting carbs, waited too long after training, or built a plan that is too aggressive. Sometimes hunger is also louder than the body’s actual energy need because the appetite system is not reading stored energy properly.

That is the nuance.

You do not need to obey every hunger signal immediately, but you also should not treat hunger as a moral flaw. You read it. You look for the conditions around it. You ask what made it louder.

Did it follow a poor night of sleep? Did it show up after a high-carb, low-protein breakfast? Did it appear during a stressful workday? Did it come after several days of aggressive restriction? Did cravings spike when meals became less structured? Did hunger improve when protein went up and meal timing became more consistent?

Those questions are more useful than “why am I so weak?”

And yes, much less dramatic. Terrible news for the motivational speaking industry.

One thing to try this week

For one week, do not just track whether you were hungry. Track what happened before the hunger.

Write down three things when hunger feels unusually strong: your previous meal, your sleep the night before, and the time since you last ate. Keep it simple. No calorie spreadsheet. No forensic investigation with a sad salad under a desk lamp.

You are looking for patterns.

If hunger is consistently worse after low-protein meals, fix protein first. If it is worse after short sleep, stop pretending sleep is a soft lifestyle issue. If it is worse after constant snacking, look at meal timing and insulin response. If it ramps up after several days of hard restriction, the plan may be too aggressive.

Leptin resistance does not mean hunger is fake. It means hunger can be biologically distorted.

Once you understand that, the goal changes. You are not trying to become tougher than your appetite. You are trying to build a system where appetite has less reason to shout.
